Swanton Community Care
The aim of Swanton Community Care is to support Service Users to live as independently as possible within their own home, and to make choices that will enable them to lead a fulfilling life where they achieve their personal goals and ambitions.
Huckleberry House supports five live-in Service Users in the Hessle based home. As well as Huckleberry House, Swanton Community Care works with Social Housing landlords to provide support for individuals who are seeking to identify a property that suits their needs, including single tenancy flats, houses and bungalows. Once appropriate
accommodation has been chosen, Swanton Community Care also provides personalised care and support to the tenant which is tailored to help them to settle in the local community.
The service offers person-centred care and support which focuses on promoting choice, independence, and skills development. Programmes of care are shaped around achieving positive outcomes for Service Users, and are tailored to meet the unique needs of each individual. This can include support for personal care, the development of independent living and social skills, and guidance on medication and other health needs.
