Swanton Community Care - Sunderland (Treen)
Tyne & Wear
Treen service in Sunderland is run by a team of highly dedicated and qualified staff, who have a variety of skills, qualifications and expertise. The team shares Swanton's values and philosophy of providing high quality, personalised care, and supports Service Users to maximise their potential. Staff are appointed following a rigorous recruitment process, and each individual Service User is offered a range of services to meet their individual needs. Once appointed, staff also undertake an accredited Autism awareness course.
We implement an innovative and flexible approach to service provision, which centres on the importance of utilising local resources to encourage Service Users to become active members of their community. We support Service Users to participate in activities outside of traditional settings, such as day services , and we aim to help them maximise their independence and reduce reliance on statutory support services.
Details about care and support
No matter what level of care is being delivered, we are well placed to support individuals to access the community and develop social and independent living skills. We take the time to understand each Service User’s needs and aspirations and to help them achieve their goals, through for example, securing a job placement.
Care plans are tailored and adapted to meet Service Users’ evolving needs, particularly as their level of independence changes. These needs are re-assessed on a regular basis to ensure that we are always providing care which is responsive to each individual’s requirements. We always seek to maximise independence whilst ensuring each individual’s safety and wellbeing.
Treen provides residential and supported living services for young adults with Autism. Support is provided round-the-clock, every day of the week, by a highly trained and dedicated support team. The team provides practical assistance and emotional support, which encourages positive behaviours and enables the promotion of community participation. Our service model is focused on understanding the needs of each individual Service User, and we work in close partnership with families and multi-disciplinary professionals to ensure that only the best care is provided.
