Swanton Community Support specialises in supporting people to live a rich and rewarding life in their own home and community.
Supporting people with a range of support needs including learning disabilities, autism, mental health support needs, acquired brain injuries and other complex care needs, we empower each person to live life the way they choose to.
From supporting an individual for a few hours each week to 24/7 care and support, each person’s individual needs and goals are at the centre of the care packages we design and deliver.

Overview
With person-centred planning playing a central role, every person is actively involved in recruiting who supports them and in how their support is delivered. Each member of the person’s care team is closely matched to the person’s support needs, interests and hobbies, and aspirations, and training is tailored to meet these needs.
Independent living
As well as helping a person to develop greater independence in their own family home, we also help people find a place to live which could be as a tenant in purpose-built supported living accommodation or in a shared house with other compatible housemates. The person is actively involved in every step of the process, and we work closely them and their network of support to identify any assistive technology or adaptations that need to be made to enable them to live independently and safely in their home.
Community engagement and inclusion
We support and encourage each person to access and take advantage of the many social, employment and voluntary opportunities in the local and wider community as well as supporting them to pursue interests and hobbies.
Delivering excellence and innovation
Maintaining high quality care and support is our priority so we regularly review and adapt each person’s care package as their needs, interests and independence develops over time.
