Supporting people with a range of needs including:
Person-centred support
By tailoring the support we provide to meet each person’s individual needs and goals, we help each person move towards greater independence and community engagement – encouraging and supporting them to access the opportunities, activities and facilities open to everyone in their community.
Flexible and responsive
Working with each person and their circle of support. we adapt the support we provide to meet their changing needs and requirements as they change and develop over time.
Delivering structured, skilled practical and emotional support to people with autism
Our autism services are tailored to the needs and requirements of the person we are supporting. From a few hours each day to 24/7 care and support, the service we provide is tailored to help each person live as independently and safely as possible in their own community – enabling them to live a rich and rewarding life.
Personalised, flexible, skilled, our autism services are based on best practice
Working with the person and their support network, we listen, adapt and make changes over time to provide the right level of support to help the individual to develop skills and coping mechanisms to increase their independence.
Helping people build a sense of self worth and purpose
Our specialist mental health support needs service provides support and recovery services for people living with mental health support needs which include schizophrenia and obsessive-compulsive disorders, as well as anxiety and depression.
By focusing on the person, not just their diagnosis, we work with each person on their journey of recovery. We understand that it’s never straight-forward – each individual with mental health support needs will transition backwards and forwards in their own personal journey.
Every person has a unique contribution to offer and we empower them to realise their self worth
No two journeys are the same and the support we provide is personalised to each person’s individual voyage.
Our aim it to ensure that each person we support has the opportunity to live independently in their home, to make their own choices and live their life in the way they want to
Our teams of specialist, experienced, skilled, empathetic and dedicated nurses respect the dignity and the integrity of the individuals we support who have suffered injuries through a medical condition or trauma including an Acquired Brain Injury (ABI).
Care plans are developed collaboratively with the individual, their family and other health and social care professionals to ensure that we provide holistic nursing care and support that meets their individual needs and requirements, whilst supporting their goals and aspirations.
Innovation and excellence are at the heart
From supporting people in inpatient rehabilitation services to assisting them to move into community based support, our specialist support team helps each person to rebuild their life and develop their independence.
Placing the person we support at the heart of the service we provide
Working closely with each person and their circle of support, we design and deliver personalised packages of care and support to meet each person’s individual needs and aspirations – whether it’s 24/7 care and support to a few hours each week.
Each member of the person’s specialist care and support team has the skills, experience and training to meet the person we support needs. As their needs change and develop over time, we adapt our training and resources to meet their required needs.
Making use of assistive technology where needed, we embrace innovation and strive for excellence in the services we provide.
