Our supported living service enables each person we support to live independently in their own home – empowering them to make their own choices and decisions and live life to the full in their own community.
Whether it’s a few hours support each week or 24/7 care and support in one of our nursing care homes we offer a truly person-centred service
Working with the person and their network of support, we tailor the service we provide to meet their individual needs and aspirations. With regular reviews and assessments, we are able to adapt and develop the support we provide to each person’s requirements and goals as they change over time.
Each person is actively involved in choosing who supports them, interviewing potential support staff, planning the training that staff will receive based on their support needs and the outcomes that they want to achieve, and designing the package of support they will receive.
Supporting people with a range of support needs including:
- learning disabilities
- autism
- acquired brain injuries
- mental health support needs
- other complex care needs
