Meet our team

Nicola Bolton

Service Manager

I joined Swanton Care as a Registered Manager in June 2022, having previously managed care homes that support individuals with enduring Mental Health and Complex Health needs since 2011. I am a Welsh speaker and in 2014 I was fortunate to achieve a Gold Award in the category for excellence in leadership and management in supported living or small group community living.

I am proud to work alongside dedicated and passionate team members, who pride themselves on recognising that Every Moment Matters to each individual we support. At Dan Y Graig - Swanton Care, we very much believe in tailoring ourselves to meet the needs of the people we support. Our therapeutic relationships are fundamental to the quality of service and care we pride ourselves on delivering to our residents, family, friends and our funders. Delivering excellence is in the heart of everything we do.

Kuba Nowak

Regional Director

I joined Swanton Care in September 2006 as a Support Worker at Maesteilo care home, since then I have gradually progressed through to Team Leader, Deputy Manager, Home Manager and most recently to Regional Director of Wales in July 2022. Throughout my time with Swanton Care, they have supported me through my Level 3 and Level 5 management qualification in Health and Social Care.

I have supported people with learning disabilities and autism for over 16 years, the knowledge and skills gained during this time is invaluable and has supported my progression through to my current role. My philosophy is to provide person-centred care that embraces safe and meaningful care for the people we support, encouraging growth and opportunity. I am a strong believer that working together works best and working in partnership with the people we support brings more positive and meaningful outcomes.

During my seven years of being a Manager, I have helped develop 4 members of my team who then progressed to Deputy or Managers. Encouraging my team to develop and grow always brings a smile to my face and I look forward to continuing this throughout my role as Regional Director.

Kate Pearce

Chief Operating Officer for Scotland and Wales

I became a Chief Operating Officer in July 2022, prior to this I was Regional Director to Values in Care for 2 years. During my time in the social care sector, I have gained a wealth of experience, starting my career as a young Support Worker in 2003, progressing through roles such as Keyworker, Manager and Area Manager.

I have loved gaining qualifications along the way, including a Level 5 in Leadership and Management and an ILM Level 5 (The Institute of Leadership and Management), this has helped me to support the people in our care to have a better quality of life. I also have a keen interest in Positive Behavioural Support and am continuing my learning by undertaking a Diploma in this specialist area.

During my 19 years in the social care sector, I have always worked in complex homes for people with learning Disabilities and autism, leading and directing teams to ensure we offer the best care possible to the people we support. Growing organically has ensured that I have vast knowledge, experience and understanding in the care sector.

About

Within half a mile of the home is the historical town of Kidwelly, where you’ll find an array of amenities for the people we support to enjoy, such as many recreational parks. With parc y bocs, a family friendly orientated facility that has a café, restaurant with full wheelchair accessibility, including gardens and grounds.

We support an array of disabilities, such as learning disabilities, physical disabilities or other long-term complex needs, such as acquired brain injury, autism / asperger, personality disorder and dementia. The people we support at Dan Y Graig - Swanton Care have access to the home's company car, which will enable people to not only access the local area, but also the wider community as part of their packages of care.

At Dan Y Graig we strongly believe in a person-centred holistic approach to care and support. We recognise people as individuals, and we will tailor our approaches and communication styles to meet the needs of the people we support. We are led by the people we support along with the individuals care teams.
