Jane Jakeman

Divisional Director South West

With over 40 years’ experience across the private and third sectors, I have dedicated my career to supporting adults and children with learning disabilities and neurodiverse needs. Beginning my journey as a Support Worker, I have gained extensive operational and strategic expertise through a wide range of roles, shaping my approach to leadership and quality improvement.

I am passionate about creating environments where people can truly thrive — places where every moment, and every achievement, matters. My values are rooted in dignity, opportunity and meaningful outcomes, ensuring the individuals we support experience the highest standards of care.

I hold a BA (Hons) in Health Studies, specialising in learning disabilities and the opportunities and inequalities that individuals may face. My academic and professional background continues to inform my commitment to inclusive, person‑centred practice.

I am dual‑registered with the CQC for our Bournemouth, Christchurch and Poole branch, where both services under my leadership currently hold Good ratings. I am proud to lead with a focus on quality, consistency and continuous improvement across our division.

Swanton Community Services – Somerset provides regulated home‑care and supported living support to people across the wider Yeovil and Somerset area. Based at the Yeovil Innovation Centre, the service supports adults living in their own homes and within supported living settings across local communities.
 
We work with people who have a range of needs — including learning disabilities, dementia, mental health conditions, physical disabilities and sensory impairments — delivering personalised care that helps each person maintain independence in the place they call home
